Forms and surveys decision
Jotform
Build a pared-down embeddable form collector yourself for simple use cases, but keep paying if you need the large template library, extensive integrations, enterprise/HIPAA features, or advanced AI/workflow capabilities.

Every project below is open source and already does this job today. Fork one, self-host it, or take the parts you need — the build prompt further down assumes an empty file, and this is the shortcut past that. Licences differ; check the one on each card before you ship.
What a replacement has to do
- Render embeddable web forms, collect and store submissions, trigger notifications/webhooks, allow file uploads, and export submission data.
What it still won’t have
- Large template library (20,000+ templates)
- 150+ third-party integrations
- Enterprise features (SSO, white-label, local data residency)
- HIPAA and advanced compliance options
- Built-in AI agents and advanced workflow automation

The verdict above measures whether you could build it. This one is only about money.
Runnable build prompt
Build a minimal self-hosted form service using React (client) + Node/Express (API) + Postgres (submissions) + S3-compatible storage (uploads) and a small admin UI. In scope: drag-and-drop or template-based form creation supporting text, email, select, file-upload, and conditional logic; embeddable form endpoint (iframe/script); submission persistence in Postgres; file uploads to S3; send notification emails via SMTP/SendGrid and deliver webhooks with retry logic; admin panel to list/search/export submissions as CSV; basic auth for admin. Out of scope: 20,000+ template library, 150+ third-party integrations, payments, HIPAA/compliance features, enterprise multi-tenant Admin console. Include input validation, error handling, idempotent webhook delivery, unit and integration tests, and deployment scripts (Docker + single VPS or managed cloud).
How we checked
How the score was reached
- Partly verdict base52
- An open-source build was found+5
- 5 cited sources+3
- Evidence score60
The base comes from the verdict. Everything under it is a check that either happened or did not, and each one is a fact frozen in this record rather than a judgement made at render time — so the same evidence always produces the same number.
